RYR 738 took off @EIN from wrong point without clearance

Incident: Ryanair B738 at Eindhoven on Oct 18th 2012, took off from wrong intersection and without clearance
A Ryanair Boeing 737-800, registration EI-DLD performing flight FR-1518 from Eindhoven (Netherlands) to London Stansted,EN (UK), was in contact with Eindhoven's ground control and had been cleared to taxi to runway 04 hold short of runway 04 at holding point F. The crew however continued along the taxiway parallel to the runway for a full length departure, lined up and took off without clearance. During climb out air traffic control made the crew aware they had taken off without clearance and they were expected to depart from intersection F, not full length.
Netherland's Onderzoeksraad announced they have opened an abbreviated investigation into the occurrence reporting that the crew was cleared to the holding point F hold short of runway 04, but taxied for a full length departure, turned 180 degrees and lined up, ran the takeoff checklist and advanced throttle levers believing they had been cleared for takeoff only to be made aware after departure that they had taken off without clearance and from the wrong intersection. There was no risk of collision with another aircraft.

Hi,
Nice to read especially because EIN is my back garden and I flew that reg last week out of CRL..
That made me think... the regs,..is there a planned path/shedule for each plane or are planes just picked by availability and is te any logic in here?
As their are a fixed planes at each beach,.. how did oo-dld made it to EIN from CRL

the logic behind it are the maintenance schedules. The plane probably went from CRL to DUB to STN for maintenance and is now flying out of STN

Don't know about specifics of the situation (visibility, traffic load, ground radar or not...), but this looks like poor controlling to me. The controller should have seen this imo. It is probably going to be a case for a Human Factors specialist. The incident with the KLM 737 at Schiphol has proven why we should wait on the final report. It is impossible to judge the circumstances of the incident from a forum:
> How high was the workload of flightcrew/ATC
> Was there other traffic that could mistakingly have been identified as this flight?
> How is the signaling on the taxiways?
> What were the prevailing meteorological conditions at the time of the incident?
The above is just a short list of questions that need to be understood thoroughly before one can even start to analyse the contributing factors to the incident.
And just for the record: inadvertent take-off from the wrong intersection without a take-off clearance is not something that should be taken lightly. As a matter of fact, it is one of the contributing factors to the most deadly crash we know in our aviation history.

For the ones who are interested, here is the Onderzoeksraad report on the B737 that took of from a taxiway at Schiphol. It is a good case in point to demonstrate that one needs to know all factors at play, before judging crew, atco's or anyone else involved.
