It's been a while since I've visited this site, but have something to report that may be of interest.
Whilst taking the dog for it's morning run, and living underneath the Sydney Airport's main runway alignment, I was at the local park and watched a United 747 take to the sky. It was making a very sharp turn to the west, which got my attention.
It was so nimble and light in it's movement. (common in Sydney, after long haul flights get in as close to the 6am city curfew, and I guess continue to Melbourne as a relatively very light craft!??!).
It's agility had me watching... sharp turn, then levelled... very graceful.
Two seconds after levelling, a flame came out of the inside port engine, extending to the tail; about 3 seconds later I heard a "boom,boom,boom" - succinct sounds in about a quarter second. Tap on your desk 3 times, as fast as you can, and that is the timeframe that I'm trying to describe.
The 747 continued into the sky unevenfully; my dog still was obsessed with chasing his ball; and I went home and sent an email to CASA, in case what I saw was relevant to anything!
I'm not sure, but I hope I saw a rare event. Those "booms" would have been terrifying to those on board.

Yes, I know how an engine failure sounds like. I once saw a heavy MD-11 taking-off runway 25R and just after take-off it made a terrible noise and flames where coming out of engine number 3. The day after it was standing again at Brussels Airport so I guess it flew to the Belgian coast, dumped fuel and returned to Brussels.
Belgian coast, if it is critical they even don't go that far. I remember a B767(thought AA) a few years ago with an engine failure just after rotation, it dumped it's fuel during the turn back to Bru. They never went to the sea to dump. I was just airborn again at Ebdt when it turned on final 25L and you still saw the fuel dumping in progres.

Long time ago, it was on an EB PANAM flight over India, (that's long time ago isn't it?) engine 2 did the same, but inside the a/c there wasn't that much noise audible. I was in a windowseat next to that engine, (why me?) and all I (we) noticed was an orange flash or two, and some fire leaving from that engine. Then we made in to CCU on 3 engines, as you meanwhile would have understood.
