Disclaimer: I can only judge airlines I've tried myself (46 so far), and service experienced. With that, in my experience:
Best:
1) ANA All Nippon Airways
2) Singapore Airlines
3) a toss up between several: KLM (overall), Air France (onboard service), Asiana (overall)
Worst:
1) BA (economy class experience - I actively avoid flying them nowadays)
2) Air France (on punctuality)
3) Korean Air (overall; particularly rude and unhelpful staff)

That reminds me: I made one flight on Shanghai Airlines from Xi'an to Shanghai (1999) and the seat next to me had no place to sit on... I could lift the seat part out of my own seat also. Interestingly, the inflight magazine made much of Shanghai Airlines being the safest airline in China. Favorite airline for me is based on some criteria like
1) check in and check in system (software). I have done check in for some major airlines (in fact I still do) and there's a huge difference in check in systems. In some systems changing seats, adding fqtv data or whatnot is really easy and then in other systems it's just hell and it takes like forever
2) Ticketing: are they friendly, can they solve problems fast (e-tkt, no e-tkt?, rerouting late pax,...) and with a smile? Do I get the feeling they know their job?
3) The flight itself: Do I feel welcome on board? Safe? Relax? Will this airline get me to my destination on time? Do I have to worry about food poisoning?
Based on all that here are my favorite and worst airlines...
Favorite:
1) Lufthansa. Really good check in system, ticketing staff is helpfull and know their job very very well and never had any complaints about any of my flights (except for that 1 FRA-BRU on a Eurowings Bae146 in 3-3 configuration - pure torture I tell ya -
)
2) Thomas Cook Airlines Belgium. Love to do the check in for them. I consider it a challenge: 180 pax, get them all checked in on time, families all seated together (yes it can be done). Everything done correctly and ready for an ontime departure. Love to fly them as well but I have some very good friends among the cabin crew so I might be biased
3) Swiss, flew them a few years back and it was one of my best flights ever so...
Worst:
1) KLM: check in system is a huge mess and the people at the ticketing counter in BRU are absolutely clueless!!! I'm sorry. Wait I take that back, I'm not.
I haven't even flown them and I hate them already...
2) Ryanair: they're cheap allright but selling people a ticket to Barcelona and then drop them off in Gerona. I'd rather go by car...
3) Virgin Express: not that cheap and they're following ryanair's bad example. Alicante south (wtf is that all about ???)

I don't know how it is in Sweden but in Belgium that's a pretty normal thing.

I guess I should explain the airlines I hate.
Air Transat: Lost baggage, never returned. Small seat pitch, and little leg room. Lockheed L1011 was loud, with rude f/a's. We were given a rude handling at Gatwick, and even worse at Toronto.
Air Canada. Wow. Speaks for its self. Rude, rude RUDE! I am imbarassed and ashamed to have them as our FLAG CARRIER! It's disgusting. I liked it better with Canadian. ANother thing is AC is very out-of-date. Most airlines are upgrading to PTV's. Not AC. Most airlines have a recent fleet. Not AC. The again, most airlines want to keep their customers. Not AC! The only time I've evr heared someone say they would travel with them again is when they had no choice as they were the only airline that serviced a certain airport.
CO gave us rough handling. Not to metion their screw up at MAN with us, they have given us poor UM services. We have been left unatended, including on a runway. They use what charters use from BHX-EWR, giving you a small, cramped, unpleasant service. I do nto like any main-stream American Carriers (excluding WN, B6, etc.), but this one bites the dust.

Based on the value for money the offer, and on the actual service vs. the expectations raised (by all their PR bla bla, inlcuding webpages); my raking is:

Best:
1- Lufthansa

3- Vueling
My worst is KLM (and unfortunately, the one I use the most)

My favorite airlines are these:
1. Swissair (Before they closed down that is....
)
2. Air Malta (Friendly and professional crew and good inflight catering
)
3. Lufthansa (Quite good overall, but the food isn't anything special)
Worst Airline, I've ever been on:
Iberia (Horrible staff, esspecially the F/A's.... Very rude
! No to mention the delays

It is easy to pick airlines that I like, but very difficult to pick three favourites in order. The three airlines I mention are ones I like equally but for different reasons. And I hope that extinct airlines are ok!
My favourites are
SNBA (nice aircraft with comofortable seats, a good range of routes from the UK so I have a choice of several airports to fly from - Manchester, Birmingham and Newcastle are all easily reached from where I live).
VLM (I love the Fokker 50 aircraft, good food, nice friendly crews and London City as a hub!)
Sabena (the best I flew with ever from the ones I remember).
Least favourite:
Only three I am afraid because all other airlines I've flown with I would happily fly with again:
Air New Zealand (bad inflight food, bad crews, bad ground staff, bad reservations staff)
Icelandair (nice crews but I don't like their aircraft).
Aer Lingus (crews very average and now gone down the cheapo road).
Other airlines I like are:
Finnair (nice crews and aircraft).
Air France (pleasant crews and always remember them for my first experience of the Airbus).
British Airways (a long time ago but they have a great reputation!)
Cyprus Airways (great flights in their 707s)
Iberia.
